The ring is probably fake. The piercing (if it was real) would be called an apadravya. Normally it would be pierced with either a straight barbell or with a captive bead ring. No bead is visible on the ring, and the bead is larger than the ring gauge, so it is unlikely that it would pass unnoticed into the piercing. (It's also very uncomfortable if it does happen; you know pretty damn fast.) It could be a "seamless" ring, but the odds are good that you would notice the seam where the ends join up.
It also appears that the model is uncircumcised. If that is correct, that would mean the piercing would go through the foreskin. That is never done, to the best of my knowledge; that would cause erections to be extremely painful, and would prevent proper cleaning of the piercing during the healing process.
