Looks to me like they're just sucking in their cheeks.
You're not looking very closely. Look at the size of the man's head, before/after.
Kind of looks to me like they photoshopped their normal faces to be puffy/larger, then had them suck in their cheeks and photoshopped them extreme skinny.
